Client registration

A bit of paperwork before we start, so your first session can be about your child instead of a clipboard. It takes about ten minutes, and you can do it whenever suits.

You do not need to find anything here

Registration happens when you book. Your form arrives in the confirmation email for your first appointment, as a secure link that belongs to you — which is why there is no general form to fill in on this page. This page is just so you know what is coming and can get your details together beforehand.

Already booked and cannot find the email? Check your junk folder first, then get in touch and we will send you a fresh link.

What to have handy

  • Your child’s name, date of birth and address
  • Your contact details, and who else we can speak to about your child
  • Medicare number, and your NDIS plan details if you’re using one
  • Your GP’s name and clinic, and any other professionals involved
  • Any diagnoses, medications and allergies
  • Reports from other professionals, if you have them — you can send these separately

Do not worry if you cannot find something. Leave it blank and tell us at the first session — nothing here is a barrier to being seen.

What happens with it

  1. Book, and the form comes to you

    Your registration form arrives with the confirmation email for your first appointment. There is nothing to hunt for and nothing to download — just open the link when you have ten minutes.

  2. It goes straight to your child’s record

    Submitted securely to Cliniko, our practice management system. It does not travel by email, and it is not stored on this website.

  3. We read it before we meet

    So the first session is spent with your child rather than on paperwork, and you are not repeating a history you have already told three other people.

Your information

The form is hosted by Cliniko, our practice management system, and submits directly to your child’s clinical record. It does not pass through this website and it is never sent by email. Health information is handled under the Australian Privacy Principles, and you can read exactly what we collect and why in our privacy policy.
